Hereby I authorise the Gesellschaft Anthroposophischer Ärzte in Deutschland e.V. (GAÄD) [Association of Anthroposophic Physicians in Germany, registered association] with the creditor ID DE23ZZZ00001060153 to collect payments payable by me at their due date, charging my account via direct debit. At the same time, I ask my bank to honour direct debits drawn on my account by the GAÄD. This authorisation can be revoked at any time. Should my account fail to show a sufficient account coverage, there will be no obligation for the bank keeping the account to honour the direct debits. The mandate reference number will be communicated separately.

Note: I can claim a reimbursement of the collected payment within eight weeks from the day of collection. The terms and conditions agreed with my bank apply.
